Dawg haru maile form ta bhare but teya form ma tala herda Lekh Nath Gautam dawg ko nam lekehya raicha pahile nai.. sunna ma ako cha application firta aucha re testo form pathayo bhane. Dimag ghumi sakyo.. k garne bhanera...
Please share your suggestion. K garu bhanera. Purai ready bhai sakyo send garna lai.

You should be fine. Your friend must have picked up the form from embassy and they pre fill those two fields these days. Only thing you have to be concern is the finger print and signature shouldn't touch any corner for the box.
For peace of mind , you can always call embassy and confirm it.
