 Question on Filing GC for spouse and child

Filing I-130 for my spouse and child.
Few questions:
1. I understand I have to file separate I-130 for spouse and child.

2. The first step is to file I-130 and once it is approved then only I can file I-485 and other forms if necessary (I-131 Travel , I765(work permit)

3. In I-130 form question 14- date and place of admission for or adjustment to lawful permanent residence and class of admission
For place - Since I filed GC through my employer.
Should I write the USCIS service center that is available at back of my green card? In my case it's Lincoln Nebraska.
or It's the city where I used to live and work(address of my employer) while filing I-485?

4. In I-130 question 17. List spouse and all children of your relative
Should I write my wife name or my name as spouse of my relative?
I assume for children it's my son information

5. I-130 Question 20 - If your relative's native alphabet is other than Roman letters, write his or her name and foreign address in the native alphabet
Should I leave it blank?

6. I-130 Question 22 - My spouse is back in NEPAL. so Should I fill up the second portion

If your relative is not eligible for adjustment of status, he or she will apply for visa abroad at the American consular post in :
City: Pokhara and Country (NEPAL)

I am confused by your question. If your spouse is in Nepal, why would you have to file I-485, I-131 and I-765. You would just file I-130 and once it is approved; start the consular processing through NVC and then ultimately Embassay in Nepal. If you are US citizen already, you would not have to wait for the consular processing as the visa are always available. Otherwise you would wait for your case number to become current and then start the consular processing. The current wait time for spouse of US permanent resident is 2 years approximately.

I think you should put the place you went for interview or the city you lived in for the place you were admitted as US resident.

Part 2 and 3 is about you. So you would list your spouse and children in this section.

Q-20- print the form and write the name and addrrss in Nepali in black ink or use word document or other online software to get the name and addrrss in nepali and copy and paste it into the box.

Q-22: list your current spouse in 20 and put NA in 21.
Leave spouse 2 blank if this is your only marriage.

I still have a hunch it's the USCIS service center rather than the city.

your reply for Q-20 and Q-22 makes me confused because the I-130 form I am filing is an old version
USCIS Note:
02/27/17. Starting 04/28/17, USCIS will only accept the 02/27/17 edition. Until then, you can use the 12/23/16 edition. You can find the edition date at the bottom of the page on the Form and Instructions.

Sorry I thought you were filing the most current form.

 If you are using the previous edition, then:
Line 17-Part C: this is your relative (your spouse). So you will put your name as husband and your son as son. For your son, you will list you as father and your wife as mom.

Line 20- answer is same as my previous one. Either write with black pen the full name and addrrss of your wife in Nepalese or copy and paste from some english to nepali converter website.

Line 22- put N/A in all four box. Don't put Pokhara, Nepal. It is not applicable in your case. Only fill this if she is actually here in US and is not eligible for adjustment of status and have to go back to Nepal for further processing.

I hope this helps. I am not a lawyer but have completed these forms for a lot of friends and family.

oh yaa one more thing:
Part C:
Line 17-  List spouse and all children of your relative

"this is your relative (your spouse). So you will put your name as husband and your son as son. For your son, you will list you as father and your wife as mom."

For 
For my wife, I added my name as husband and my child name as child

But For my child,
Since my child doesn't have spouse and children, I think I have to leave it blank instead of listing myself as father and my wife as mother.

Yes, you can leave the section blank for your child's I-130. I am not sure on the place of admission. I have been putting the port of Entry as place of admission for people who came here with Immigration visa or N/A for people who have already become US citizen.
