4. Address: 75/14 Unmatta Tole, Purvi Golardha
5. Medical Record Number:977
6. Attending Physician: Dr. MeroDeurali
7. Diagnosis: Sepsis
8. Description of the Pt.’s condition: At the time of her admission at the Emergency
Department of the Deurali Hospital, the following symptoms were observed:

a. Increased Fever
b. Decreased Blood Pressure
c. Severe Weight Loss
d. Shivering
e. Signs of lacerations and bruises on her inner upper thighs
f. Right cheek swollen, bluish in color

9. The battery of test ordered by the attending physician:

a. A Complete Blood Count
b. Comprehensive Metabolic Profile
c. Chest X-ray
d. Blood Type and Screen
e. Microbial tests

10. Critical results reported back to the attending physician:

a. Hemoglobin 5.5 gram (decreased oxygen carrying capacity)
b. Organisms detected:

a.Immediate transfusion of eight units of blood cells at Dialysis to remove microorganism from the system
