no they don't write parent's name 

They write all name/dob etc at invitation letter......it might depend college to college....but they wrote all name....dob (and other info like address etc just to make sure he/she is the right person) when i requested. good luck.

I think you did not request, upon your request they do every things also they give you seperate foam and you have to fill up every thing, if you need special explain in your letter you have to fill up in others column.
