You have to be enrolled in one of the residency programs even though you pass your USMLE. Instead of going for an elective, apply for masters instead (so that you can obtain a student visa). Once you arrive, you switch into elective like health Science and apply for residency. Remember, you need to complete your residency even though you pass USMLE! You will then be able to practice medicine.

Dear hariraman,
dimag kharab jyu le alik kuro bujhnu bhayena jasto cha!
First answer these questions to yourself,
1. Which year into medical school are you in right now? ( useful if you are into your fourth year)
2. Have you taken any of your USMLE steps yet? ( atleast taken your Step 1)
3. Do you have any english proficiency tests? (Better if you have TOEFL etc)
4. Can you afford the costs involved? (Travel costs and some programs have few thousand $ fees as well)
Here is what i would like to advise you.
1. Take atleast USMLE Step 1 (And don't forget to score 99 percentile).
2. Take TOEFL to increase your chances into better medical schools.
3. Start communicating with the medical school elective co-ordinator.
4. If you know anybody from a medical school (be it anybody janitor to the dean) request them for an elective and show your future interests.
5. Google medical school you prefer or geographic area you prefer. (If you need help start with : FREIDA online service).
6. Focus around NY, Florida, California, Texas area medical schools. Heard they have a high acceptance rates. Also be aware about the fact that the US citizens who attend carribean schools do rotations in NY and FL area.
This is not an easy task. And its very likely that you will not get accepted for an away elective unless you really know someone. But if you follow the above route you increase your chances. And i'm glad you are aware about the fact that how tough it's becoming to get into a residency program for an IMG. Away elective into an US medical school will create a stellar CV (this is neither an observership nor externship, you are like US medical students) and you increase your chances of getting into a decent residency program and the speciality of your choice.
Wish i could help you more.
Well good luck!

Dude, i'm afraid that most likely they will not consider you as a medical student any more since you are already into your internship phase (even though you have not graduated yet). This is the trend but i might be wrong. If you are considering it very strong, talk directly with someone from the medical school you are applying into and try to convince them that you are still a medical student and have not started your residency yet. Otherwise i would suggest you opt the regular USMLE pathway and consider applying for research/extern/observer position. Overall the results are not that depressing for Internal Medicine. What speciality you wanna go into? Hope you are not thinking about geting into Neurosurgery, ENT or Orthopedics. If for Internal Medicine you just need to prepare yourself, get good board scores, have some patience and you will be just fine.
Honestly, in your case, i think you should just choose the usual route and forget about doing a rotation here. Take the boards, score high, good if you get some clinical experience in the US and probably be able to start residency in June 2012.

thank u for the suggestion.... i had also thought the same.. but here in My college, and particularly in India, all the USMLE aspirants are actually applying for electives in their internship....This trend is quite high here..... and also this gives them an edge in their residency match.... Thats why i m too opting for this.... And they say that externship is much easier if u know some doctor who is actually working there..either u can work under himm... or he may recommend u to someone....
tyai bhara i m appealing for this help to all the doctors in USA...
